I just had this exact issue.
You need to add in a check for if the directories have write permissions and alert the user if the file copy didn’t work. Some hosts don’t use suPHP and you’ll probably find that the copy is being done as user ‘nobody’ instead of the account holder.
I had to manually move the offline-credit-card.php file with FTP as epschmidt mentioned in a post on this thread.
